Hi,
It seems like you lack the Create privileges for incident entity.
Go to the form editor, and click Navigation
Make sure you have Cases/Incidents on your left-hand column.
If not, you should see it on the right-hand side in the Relationship Explorer. Simply click and drag it over to the left column. Save & publish.
