Hi,
We are running CRM 2016 on premise.
On one server we have 3 identical test instances - on only one of these instances the deactivate accounts is not working.
The button is showing, when you click deactivate nothing happens and there are no errors.
- There is no javascript on any forms
- it reacts the same from the form as it does the list view
- all services are running
Can anyone suggest anything else?

As Andrew already told, you can always check why it isn't working with the developer tools in your browser.
But keep in mind some other things:
1st - Does that happen with only one browser or all the browsers? Updated browsers?
2nd - Do you have your CRM properly updated?
3rd - Does that happen to all the users or to a particular user?
4th - Do you have any customization on the button not working that may lead to this?
If you see that nothing of this makes sense, you can always open a case with Microsoft.

There are 2 ways to resolve that issue:
1. Do it yourself - you will have to find the reason using Browser Developer Tools. In order to do that you will have to find JavaScript function that is called onclick on deactivate button and find out what's wrong with function.
2. You can let Microsoft to do that for you but you will have to open ticket with them.
