Dynamics CRM portal Column Name and Data Hidden

Hello all.

I encounterd strange issue on lookup on Dynamics CRM Portal webpage. On clicking on lookup it open modal window which is supposed to show two columns Index and Name but it shows only on column which is on the left side the right one is getting hidden. If I navigate to next page in lookup window now it loads data in the second columns and even then column name is not showing up. On checking browser console found that second columns style is display none.

Does anyone encountered this issue or any info on how to fix this display none? ...
