ACA - bug fix needed for when coverage is waived for a self-insured plan

Hi, 

When a self-insured plan offers coverage to an employee and the employee waives coverage (i.e. codes 1E/2F for lines 14 and 16), the system is defaulting to "covered" for an individual and the boxes get checked (erroneously) in Part III.  The individual is NOT covered, but has been offered coverage.  Please help to get this fixed.

    Part III of the 1095-C is driven by data created from the Dependents/Beneficiaries window. Specifically, the "Covered/Not Covered" indicator. If your part III's are not populating correctly, you should take a look at the data in the UPR00904 table. Editing this information can be a pain because the "Effective_Date" value in the table is based on the User Date at the time the record is edited.

    I put together a free one-window utility that will allow you to more easily edit the data in this table as well as the UPR00905 table which drives Part II (lines 14/15/16) of the 1095-C.
