Figure out Permissions - Field Visible on form, but not able to use it?

I have a field on a form that I cannot determine how to make available. The field below has only a line shown on the Portal page. It's the Contact "primarycontactid" field I am showing on a default Case form. Finally, I am showing the Entity Permissions screen where and I am logged in as an Administrator and opened all of the account and contact permissions to "Yes."

Any ideas? I appreciate the help. Thanks
