Customer Voice no way to select the PowerApps environment when creating new project

I did the initial install which was to the default environment.

I then installed it into the specific environment where I need it.

When I login to Customer Voice and to create a new project, I select the Blank template, according to the instructions there should be a "Next" button which opens the environment select panel.  There is no "Next" button just the "Create" and "Cancel" buttons.

Anybody seen this problem and know the solution
