Custom Forms not displaying in GP 2015

Hey all,

I am customizing a form in GP (nothing too serious, just literally adding a line of static text and a button). When I access the form file through either modifier or Dexterity the file correctly displays the customization's I have made, however when i open the window in GP afterwards it defaults back to its original appearance. I've tried all the standard IT stuff, logging out, running as admin, restarting comp etc. I am also logged in as a Poweruser. I've checked and verified that the .set file is correctly routing to the Form.dic file and even noted that the Form file displays as "modified" the instant I save the changes in modifier or Dexterity.

As someone fairly new to GP customization (literally picked it up as a side project in order to lessen our outside contact needs) i'm sure I am just missing a setting somewhere. Does anybody have any ideas? Thank you for any assistance you may provide.

You need to set the access in Alternate/Modified Forms and Reports (Administration >> Setup >> System) to the modified version.
