Reason:
This occurs in specific software builds (10.0.41 and 10.0.42) where the 1095 C forms are not generated. The problem does not occur in build 10.0.43, indicating that the underlying cause is related to the software version being used.
Resolution:
Latest modifications in generation on form 10.95-C were included in version 10.0.43. But these changes have been backported to versions 10.0.42 and 10.0.41. Here you can find the build numbers of quality updates where this form is updated: Details for issue 984484
