Is the above order of permission set accurate?
 
From this the user is expected to have access to create users and add permissions, but it's not happening.

    We may need more information to help you. The order that the permission sets are in won't be relevant. What they do need is the correct permissions in any permission set. 
     
    Can you double check the User card for that user and see if the permissions are applied to just a single company or all companies. If you want the user to be a super user with permissions to create users and update access you need to make sure the permissions are applied to a "Blank" company like this:
     
    It's important to note that the SUPER permissions gives the user access to practically everything in the system. You may also want to provide more granular permission sets if you want to create restrictions for that user.
