Approval Entries notification showing different for customized page

We have created a customized Purchase Request page
 
 
 
So on sending the approval request the email being sent out is like this which is missing fields data
 
 
Whereas the same word layout is mapped for email notification sent on purchase header too but the email sent out is like this
 
 
So please guide what is the issue that's causing the difference.
