This simply means the Dynamics 365 / Power Platform environment is locked to an Azure AD security group, and your user account is not a member of that group, so access is blocked by design (notMemberOfOrg). There’s no issue with the app or URL—an admin must add your user to the environment’s assigned security group (via Power Platform Admin Center → Environment → Settings) or remove the security group restriction; once added and synced, access will work.
