Dynamics 365 app for Outlook error loading control message

Hello,
 
In a Microsoft cdx environment I am preparing to showcase the outlook integration I am seeing an error message as shown below. I believe follow all the steps to do this properly but I cannot figure out why this is happening or how to fix this. Is there anything else I can try to get the Dynamics for Outlook app to work?
