I am trying to set packing slip report to below pre-printed layout but having no luck. Please advise how I arrange sections and report header and footer to achieve below layout.

You have to make changes to the report layout using report writer. Try to change the existing format of the GP report and add required fields at appropriate positions.
From GP navigate to Report writer. Add your report from original reports to Modified reports and make changes in it. With this you can make your packing slip report with pre-printed layout.
