associate incoming email to a custom entity ?

I'm looking for architecture ideas about how to associate an incoming email to a custom entity , using the email subject : If the subject was '941' , then look for custom entity with name 910.

Workflow or Plug-in ?  What stage in pipeline to use ?

    workflow. Register on "create" of email, check if subject equals (or contains?) 941. If true, update record (set regarding to 910)

    Is there is a API to let me associate the email with the instance of the custom entity ?

    Similar to setting the 'regarding'  listbox of a opened email in the UI.

    Not sure if I really understand your question.

    When your Custom Entity is enabled to have activities associated, you can easily select the respective record for the regarding field in the update step of your workflow.
