Can someone guide me how to create app in Appsource for Dynamics 365.

You will first have to register and submit your app filling out the form and providing your contact and app information.
appsource.microsoft.com/.../list-an-app
Microsoft will then contact you with further information. Once approved and any questions are clarified your app will move App Submission to App Evaluation. Once Microsoft evaluates the app, they will provide you with instructions on how to Publish your app to AppSource.

From the information provided by you it is clear that i need to approve my app from the Microsoft and create the app according to the guidelines from Microsoft, but i need assistance as how can i create this app and from where.

Dynamics 365 app is nothing but a custom solution which can consist of all the possible component which a solution can have. So if you have some idea/functionality in mind that you want to use in your app, implement those using possible components (entity/webresource/plugins/workflow). After that you can release solution for Microsoft approval.

I think Mahen already answered this, but basically you create a managed solution that you can deploy onto Dynamics 365. Once you have that solution built and tested, you will have to start the process of getting approved by Microsoft.
The first step is creating the solution. There is no need to involve Microsoft until that is ready to go.

I think now I am clear about the fact that it is a managed solution which could be available for all Ms Dynamics CRM user once approved from the Microsoft.
