Is it possible to setup some kind of discount, that will on work once a customer has spent a certain about of money. E.g. Spend £20 and get an item at a reduced rate.

Not out of the box. look for a Gift or Points add-on from Retail Hero, Jean Holld, Retail Realm, I sure there are more.
