Why do Accounts and Contacts become inactive and get re-assigned randomly

Hi guys

I do not know if anyone else is experiencing this, but I have noticed that some Accounts and Contacts become inactive and get re-assigned randomly. Even other members in my organization have been experiencing the same issue. Has anyone else experienced this issue and if so, what is the solution if there is any or at least the recommended solution
