We are running both CRM 2015 on premise and have adxstudio's portal solution installed. I'm not exactly sure when this broke but my Case Satisfaction Survey email notification link no longer loads the Case Satisfaction Survey web form properly. It now returns the Record Not Found Message. I don;t know what changed to cause this and went back through the original forum posts from adxstudio's web site. The below URL is what I'm generating and I know it was working within teh last few months as I have completed surveys in the system.
I even went back and tried links from the old email notifications on completed surveys and I get the same message. I've gone as far as deleting the web step associated with the web form and rebuilding it. My next step is to retry building the entire web form and re-associating it with the web page. I verified the identifier request that is being generated through the work flow is accurate and while I generate an identifier request for the case, it isn't used since the survey url has the actual case number in it.
