Escalation using Managerial Hierarchy

Hi,

I have configured the escalation process using the Managerial Hierarchy.
I want my workflow to simply escalate into the next level up using hierarchy, if an approver did not approve on time.

Hierarchy is: "A" reports to "B" , "B" reports to "C", "C" reports to "D".

The issue I am facing is that when requester "A" submits a workflow, it is initially assigned to "B." If "B" does not act within the specified time, the workflow escalates directly to "D." If "D" also fails to act within the time limit, the workflow gets automatically approved. 
Again the same workflow is assigned to "C," it again escalates directly to "D" if "C" does not act, and if "D" fails to respond within the time frame, the workflow is automatically approved.

I would appreciate it if someone could guide me on this issue.
