Dynamics 365 Online. "Record Is Unavailable" error message when clicking new Lead button

Hi experts, 

I am using dynamics 365 online (V9.0). I just completed install of MD365. While clicking on New button of Lead function =>  I get the error as the below picture. 

I tried to check other function as opportunity, it work ok.

Any ideas how to fix the error please?

    Hi,

    As per the error message, you had some form which no longer exists.

    I believe if you try it on ingonito mode/inprivate mode, it will work.

    I just install app from ms. no customize.  I don't understand why ms release with error version. Could you help me how to ingonito mode/inprivate mode?

    If you do a right click on Chrome/ IR, you will get an option to open the respective mode.

    You mentioned you install app, which app you install? And did you install it on existing Dynamics v9?

    I installed 4 app on Dynamics 365 (sales, service, field service and project service).

    I already try it on private mode, it's ok. But i cannot guide end user work as it. Could you have any ideas for this?

    I don't think it would be an issue for end user.  The issue you are facing is due to browser caching i.e. when you initialy access your lead, browser stored the id of the form used and when you installed these app, I guess that form got deleted and updated forms are added to your crm.

    Now when you are accessing your lead form, browser is trying to load the previously saved form which no longer exists.

    Accessing it in private mode is not a fix, it was a test to confirm if this is related to cache or not. It will be fixed for you in some time when the cahce is refreshed or you cn clear cache manually.

    As your users haven't access the old lead form, they must not get this issue. You can however ask any of your existing users to try it and see how it goes.

    You can search web on "clear browser cahce"
