how to st up fields based on selection of Option Set

I have a project that im trying to add some fields when selection an option set,

so when i select any of the options from an option set i would like to see some fields to populate based on what option im selecting.below pic will give you an idea about what are those fields.

what would be the best way to achieve this ?

you need to write Javascript to achieve your requirement (you can try first with business rules and check if you don't find any limits on what you need to do)

Download the SDK and go to this path SDK\SampleCode\JS\FormScripts, you will be getting the JS reference for your requirement.
