Custom Button with Dialogue box

Hi I’ve created a custom button using ribbon workbench for Dynamic 365 online. When the user selects the button, I want a box to open up asking the user to answer a few questions, select a few drop down options (dynamically) and check a box for confirmation.

Any recommendations? PCF controls or custom page or something else? Thank you!
