I'm trying to do 2 things with no avail :
1- I have my BOM setup with a UOM different from Base UOM, on entering the production order in release prod. orders and quantity then refreshing the orders the output is generated by base UOM and the same quantity of order header
2-I want to add a fixed cost per unit of output in UOM of BOM, I did add a setup time on work center with cost per unit, when entering the order the cost is calculated using Base UOM

Below is my test from Prod. Order Item, to BOM, Routing, and Component Items.
Item to Manufacture:
Routing and BOM:
Prod Order:
Box of 10 = 24 LB’s.
1LB = 2.11 manufactured
1 Box = 50.64
10 Boxes = 506.40
Capacity = .11 x 24 x 10 or 26.40

On the Production Order, you are not defining the UOM you are producing until you get to the Production Line entry. So this is normal processing in NAV. I will tell you that some Add-ons expand on this depending on industry like Food that use Batches and create packaging UOM.
On your second question, it sounds like you want to add $2 per unit produced, period. I will offer up this answer:
On the Work Center, use UOM to Unit, so it will add the Cost set on the Work Center at $2.00 to each produced/output Item.
So if I produced 10, my cost would be 10*2.
Test this in your company and you will see the results.

So this is the default behavior to disregard the BOM UOM and output in Base UOM, strange for me. but who am I to argue ;)
The second question is still strange so I want to add 2$ on every produced case using the work center, when I setup the work center with 2$ per unit and production line is per case the capacity cost is showing 2*quantity per base

Yes, the Production Order Line will insert with the Base UOM for production since the Item Base expects you to output a base into inventory. The Components are based on the CASE quantity per. If you want to actually produce the CASE and put that UOM into Inventory vs LP, then when you Refresh the Production Order the Line is inserted with LP. Change the Line UOM to CASE and Replan the Prod Order. Here is my image:

My base UOM is LP , production UOM is CASE, my BOM is in CASE
I expect the production order line to be in CASE it comes in LP
On the work center the Capacity cost is coming by multiplying the cost per unit by quantity per base UOM not what is in production order line

A few setup questions...
1. Have you setup the Production BOM unit of measure within the Item Unit of Measure list for the Item being produced?
2. On the production BOM Line the Quantity per field shows the quantity of the component item in the production BOM UOM, yes?
On your second question, are you trying to add an overhead or indirect % on the produced Item? On the Work Center, if I have a Direct Cost and it takes 1 minute to manufacture 1 Item for output, this Direct Cost will be applied and posted to Item produced. I can add more cost like overhead or indirect %, onto the Work Center, to be posted. The work Center has a UOM as well so my cost will be applied by Time or Units. Time being how many hours worked to produce the Output or Units meaning how many Units produced.
