Import a statement template from another instance/installation of GP 13

I was trying to create a statement on blank paper template for word. In the process I accidently deleted the standard template. My word template comes up with a syntax error and only prints page 1 with no footer information (ie Totals and Aged balances.) How do I import this from another instance/installation of GP so that I can start over from scratch?
