Work Order from a maintenance request ( Asset Management Module )

Hi Everyone.
I'm trying to create a Work order from a Maintenance request in the newly released Asset Management module.

I'm facing the attached error. I don't see any "Name" Field on Maintenance Request --> Work Order creation form.
Above it there is this ledger validation error.

I'm using the demo data company and have re created work order Life cycle models and states.

Any help would be highly appreciated, Thank you.
