What could be causing such a huge discrepancy in the quantities?

When you run your purchase receipts report for this item to which quantity does it agree? Have ever run inventory reconcile? Make sure you have a backup first.

When was the last time your ran an inventory reconcile (even just against this item to see if there are any discrepancies that GP would fix).
Inventory >> Utilities >> Reconcile
Run it against the single item only and see if it corrects it.
Have a full backup of your company DB first and make sure no one is in GP while you're running this.
