How to? Enabling Outlook OWA in Office3 365 with Dynamics 365

I am having a world of issues trying to enable OWA to work with Dynamics 365. I have enabled the roles within Dynamics CRM and followed these instructions but I do not see the Dynamics plug in inside of Outlook OWA? Anyone have any further instructions? I want the light weight application to show up. Thanks!

I followed these two instructions:

    Ok, I managed to get it to show but I am still receiving this error from this page.

    support.microsoft.com/.../3124910

    I verified that I have the following permissions set as well based on this page...

    technet.microsoft.com/.../dn946901.aspx

    Still, no luck?

    I continue to get this error but my Organization is on and alive:

    Help me resolve this issue

    Go to control panel>>Credentials Manager>> Remove any credentials related to CRM
