Recently, around 5/6 (at least that is when I was notified of this issue from an end user) our lookups started showing the following error message: "error loading control" as soon as you click in them to populate it. It is not consistent and not specific entities. It is also not isolated to one environment (dev, stage or prod). It is also happening in out of the box entities / forms / fields and custom entities / forms / fields. I have never come across this error before in CRM. I was able to download the error log:
