Database ERD for a Medical clinic CRM , or just ERD for CRM , I am so stressed , anyone who can help it would be greatly appreciate

HI all,

I m doing a Masters in Information Science at Texas A & M international ,

and I need to do an ERD for the CRM part of the system of a small hospital / medical clinic , I have the template , but I am not able to think

what entities /objects I need . I need to do the Enterprise level, which I thin it is ok, but the project level I am completely lost
