Custom Workflow not woking for multi level user

I have created a custom workflow for the request approval of customized page, although when i send request for the specific user it is working fine but when I select workflow user group in approver type and after approving by all the users status is still pending. please suggest how to approve the document for the multi user approval.

Approval User Setup

Hii EveryOne.

As per Requirement I get a snapshot of Approval User SetUp. In that the limit and approval chain is defined . you can setup as per requirements IN email you need to defined the name of the respective users.
