Creating new menu item for GP using visual studio?

I have not been able to find a way to create a menu item in visual studio. could someone give me some assistance. just something simple like Cards>Sales>Test. Which contains a form created in visual studios.

    You use the visual studio integration toolkit to do this.

    There is no native way without this extra dictionary that exposes the required dexterity methods. It is free but you have to register keys.
