XrmToolBox returns an error when establishing a new connection

I tried to establish a new Connection in XrmToolBox with the following Server information. 

Server Name:  crm5.dynamics.com

Use SSL, use CRM Online and use Office 365.

I supplied my login account and password (which are correct as I used them to login to CRM).  However, when I clicked the Get Orgs button, I received the following error:

An error occurred while retrieving organizations: An unsecured or incorrectly secured fault was received from the other party.  See the inner FaultException for the fault code and details.  Inner Exception: Authentication Failure

    the password contains special characters like ' or " or = or ; ?

    It contains alphanumeric characters only.  the site was set up by our system administrator, we are all new to crm.

    I think it is due to 2FA login requirement from our customization.

    Do you use 2FA login as well?  Please confirm.

    I check the website, the xrmtoolbox does not support multi-factor authentication currently.

    I switched back to the on-line trial as my SA is not comfortable to give me a non 2FA login ID.

    The crmsvcutil that generates early-bound entity class also does not support 2FA login.

    Hello Tiong,

    This issue may occur for any of the following causes:

    1. The operating system date on the client computer is not set to the correct date.
    2. The operating system time on the client computer is not set to the correct time.
    3. The operating system time zone on the client computer is not set to the correct time zone.

    Please check this KB for more information, it's about the CRM for Outlook but the error message is the same.

    I'm having the same issue.

    I suspect its because of 2FA, but I don't have a solution for this.

    Has anyone found a solution?

    Found the solution, created an app password in account.activedirectory.windowsazure.com/AppPasswords.aspx and used that password to login

    This works great: created an app password in account.activedirectory.windowsazure.com/AppPasswords.aspx and used that password to login

    Then I had to give permission to both IIS_IUSER and NETWORK SERVICES account full access to the portal's AppData Directory in IIS.
