I notice that the Base Language is diabled in Settings > Options > Languages > Base Language
How do we modify the Base Language or do we setup this initially in CRM 365 on premise?

You need to install language pack first to change the language-

Once the base language is set up within the initial installation it cannot be changed.
technet.microsoft.com/.../dn531140.aspx and technet.microsoft.com/.../mt812276.aspx tells you how you can add additional languages.
During the installation of CRM on premise I don't see the option for the base language?
The base language of the first organization is tied to the language of the server installation.
