Application Ribbon Customization New Button Lable not getting change

Hi Experts,

I am newbie in CRM, in Application ribbon I have added a button, defined EnableRule, CustomAction and Command in Customization.xml and imported the solution. New Button appearing there but Lable and Tooltip description is showing "Custom". I have re checked all customization done by me but didn't find any clue. Refreshed the form, Published and cleared cache/history properly.

Please suggest but can be the reason ?

Open the Ribbon Workbench and check if these 3 fields are correctly entered.
