No customisations were made to the Qualify button, just field and form customisations to the Lead entity. How do I go about troubleshooting this?
PS: All other buttons work just fine on the command bar and the Qualify button works if I exit the lead form and click it from the list view.
Solution:
- The solution to this in previous version (older than 9.0) was to ensure that the "Company" field was on the form. This fixed the problem.
The issue resurfaced recently (23 April 2018) and I found that the solution to it is a simple update by the support team to your DB version. So if you go to the About Section of your instance (top right click the settings drop-down menu and you will see it) you will find the versions like this:
A) The versions are similar and the button works in this case.

Version 1612 (9.0.2.54) (DB 9.0.2.53) online
B) The versions are different and the button doesn't work in this case (or other mismatch cases)

So to solve, check your versions, raise a ticket and the update will fix your issue (it could also fix other issues if you have any).
Further update:
Some visitors to this post claim that the version updates may not be the issue. While this maybe true in some instances, it has been found that the regular updates rolled out by Microsoft (or their partners) do cause issues similar to this. So checking the version is always one possibility.
This is also true for other issues that may arise. In other words, if you see a problem that is very strange, chances are it is caused by the regular updates being rolled out to the system. Bugs exist in the system and are still persisting in Version 9.0.
