Customize Dialog Process Look and Feel

Could someone suggest if it is possible to customize look and feel of the dialog process that I created?

My requirement is to create my custom dialog which has only one page and only 2 buttons. On this dialog will be 3 fields as 1 checkbox, 1 dropdown option set, 1 text field. 

When I try to use Microsoft process configured as dialog , it has many sections such tips, comments, next button, summary etc which I dont want to show to the user.

    Hello,

    You can't update look and feel of this but you can create a webresource in HTML and show it on button click from form or where from you want.

    Hope you can understand what I mean

    @Jagmohan : Thanks for your response. My ultimate goal is to replace Resolution pop up which we get when click on Resolve Case button.

    I followed instructions from below page

    But when I click on Resolve Case button I get 2 page dialog of microsoft which has all the button like Summary, Next, Cancel etc.

    If you suggest me to use html webresource, then I dont have to use this dialog, right? Please advise.
