Any suggestion on dept and category creation for apparel store?

Any suggestion on dept and category creation for apparel store? Looking for some ideas for better practice when creating dept and category that can combine ILC that will be meaningful. We are starting to use color and size with matrix and running into many problems with existing old items.  The dept and categories may be setup wrong. Meaning categories are not unique they are duplicate in several dept have the same categories codes.  So I want to surgically fix the problems.  I have exported them out to excel and was going to sequentially rename them.  If anyone has any expertise in this area. I would really appreciate their input.  Thanks
