Is there a way to run a Smart List that would show me all open orders including the purchase order comments from the circled note in the attached picture? I am using GP 2013.

If you are on GP 2013 R2, you can add the Note Index field to the standard Open Purchase Orders* smartlist favorite. You will then save that as a new favorite. You can then use SmartList Designer to modify the new favorite and include the Record Notes Master table. You will then relate these two by the Note Index field.
