Dynamic 365 App for Outlook not syncing and tracking meetings

Hello, my work uses the Dynamic 365 App for Outlook to track meetings to Dynamics 365 as appointments, but the plugin has failed several times and does not seem very consistent. Some meetings will fail to track immediately while in some cases, meetings that were successfully tracked will stopped working and become not tracked again. Sometimes, it will display the message "Recurring appointment occurrence cannot be tracked" even though another series of recurring meetings are successfully tracked.
 
This occurs on both Mac and Windows, in Old and New Outlook, as well as on web browsers.
 
Has anyone encounter this or similar issue with the app and if yes, how do you resolve this so going forward the app will track all meetings without issue?
