There is a custom entity when i am trying to re assign it to some user getting this issue. I have checked with all custom logic but did not find any clue.

I am yet to get the log file from User. But i believe there will not be much information.
No that is not a queue or activity type entity.
Yes we use Outlook client for tracking Emails/Appointments, But i believe these features will not be responsible for throwing that error.
It's in production, so i can't disable plug in.
And i am unable to replicate the same issue in lower environment.
Can you download log file & see if any more useful error messages are there.
Is this some queue enabled entity/activity? There may be another queue item available already.
Do you use Outlook client for tracking Emails/Appointments?
Can you disable the plugin if you have on Assign/Update message to pinpoint the issue rootcause.

I am assigning it to a different user who have all the right to own that record. no it's not happening for all records. As of now i have found one record.

As per error message seems you are assigning same user which already assigned . Does its happening for all user ?
