Migration Table Mapping

My colleagues and I are attempting to migrate from Business Central v14 On-Prem to SaaS latest version. The data for the core tables is migrating successfully, but we have a few extensions with custom tables and table-extensions that we want to include in the migration. We've had a number of attempts but we can't get the data for the extensions to be included in the migration. We installed the extensions at the SaaS end prior to migrating the data, to ensure that the custom tables and fields exist at the destination. Additionally, we populated the Migration Table Mapping page as per the screenshot below, with no success. Any idea where we are going wrong?

    Thanks for your response. We'll try that. Also, I have 2 further questions:
     
    Does the 'Source Table App ID' column need to be populated?
     
    Does the 'App ID' have to match at source and destination? For instance if you have an extension on the old system called SampleExt14 and SampleExt23 on the new system, the table schemas are the same but they have different App ID's, will that work in the data migration?
