Problem with configuration of Outlook 2013 CRM 2013 plugin

We are using an on-premise version of CRM 2013.  We recently upgraded from CRM 2011 to CRM 2013.  My Outlook plugin worked fine for CRM 2011.  When upgrading to the 2013 plugin, I ran into errors similar to that which I had for CRM 2011 (https://community.dynamics.com/crm/f/117/t/121946.aspx), which was solved (https://community.dynamics.com/crm/f/117/p/122503/259557.aspx#259557).  I have tried similar methods to no avail.  If I remove all credentials and try to configure the plugin I get the bogus generic credential created (Microsoft_CRM_http://server:port) and the error "Caller was not authenticated by server".  If I wipe all credentials, and sign in once via Internet Explorer and save proper credentials, I get the same old error from CRM 2011 plugin below.  However, the issue before was visible from the Security log in the event viewer on the server side.  What was happening was that my local domain was trying to push through the system with logging on the other domain that the CRM was on.  Now, when I view the security event log, I see the other domain logging on correctly.  I've ran through all the steps in troubleshooting and in my post from CRM 2011 and nothing has solved this issue.  I would appreciate any community help so that I do not have to hire this out again.

    Hi,

    The Exception says an error has occurred and server was unable to process request. This is quite generic and doesn't give much clue of the problem. I would enable CRM Server side platform traces and look for errors at the same timestamp.

    After receiving support from a Microsoft Partner - The issue was found to be that the CRM server was using a name and not IP address.  When creating credentials for the system, credentials were created and saved from IE using the IP address.  This mismatch caused the failure.
