How to create a check box

Hi ,

I am adding a new field and was trying to select check box, so I selected Two options but I am not getting check box option.

    Hi,

    Add two option field on Form...Select change properties and then go to Formatting tab select Checkbox as highlighted below.

    anyone know how to achieve this on a Business Process Flow (Add check box?

    The field property is only available if you click on a field in a form.
