Using ActivityParty List for CallTo in Phone Call Activity

Hi experts,

I'm trying to create a custom phone call activity every time the user saves the form (onSave). The activity is being created, but in the title it shows "Customer Not Found." 

How can i use ActivityPartyList entity to set up the CallTo field so that it displays relevant information in the title? Kindly help me out with the syntax of the same, as I cant seem to understand how to write the syntax for that.

    Do you create a phonecall on codebehind (with plugin / workflow) or on clientside (with javascript or call any webservice)?
