Hi Shahbaaz
Do you have more information on your environment?
Are there lots of 3rd party solutions? Do you have many customisations?
Depending on these factors I would recommend a route for you
Hi Mark
We have 3rd party solutions and customization also.
As long as i know, if you want to upgrade to CRM 2016, at first you need to upgrade to CRM 2013. I think you need to do the same thing if you want to upgrade CRM 2011 to CRM 365
Take a look at this link:

Hi Shahbaaz,
You can’t directly upgrade from Microsoft Dynamics CRM 2013 or Microsoft Dynamics CRM 2011 to Microsoft Dynamics CRM 2016. However, you can upgrade a Microsoft Dynamics CRM 2013 or Microsoft Dynamics CRM 2011 deployment in-place by using a valid Microsoft Dynamics CRM product key or migrate by using a Microsoft Dynamics CRM 2013 trial product key, as an interim step in the upgrade process.

Hope this helps.

Hello Shahbaaz
Tenfold can help in integrating MS Dynamics 365 and enabling click to dial in Outlook. Our click to dial also works on webpages opened in Chrome. They also support MS Dynamics integration www.tenfold.com/.../dynamics
The route for upgrade of CRM 2011 is:
CRM 2011->CRM 2013 SP1 -> CRM 2015 -> 2016 -> D365
However major hurdle will be in first step when you upgrade CRM 2011 and CRM 2013.
There will lot of feature that need to be checked. For example in 2011 we could use the deprecated web service but CRM 2013 wont allow that. There are tool available which will help you in getting crm 2011 to crm 2013.
blogs.msdn.microsoft.com/.../microsoft-dynamics-crm-2011-custom-code-validation-tool-released
technet.microsoft.com/.../hh699673(v=crm.7).aspx
Once on CRM 2013, upgrade to 2015 will be much easier with almost no hitches.
Upgrading from CRM 2015 to CRM 2016 will again involve effort in changing javascript to use WebAPI. From 2016 to D365 is also smooth process.
You can also go through the blog suggested by other to get detail ideal.
However please take into consideration the fact that a LOT has changed from 2011 to D365. A upgrade process will not upgrade the business process which can be optimized to greater extent using D365 feature. So do check if re-implementation with upgraded design better suits your need or not.
PS
How much time did it take for you to upgrade. Also, did you guys do it in-house or had a vendor help you?
Hi Mark,
I have similar question and would be grateful if you can shed some light:
we are trying to upgrade MS dynamics 2011 to Dynamic CRM 365 (version 9.0) On-Premise, we know that we need to go to 2013 first, then to 2015 and 2016 ... I would greatly appreciated if some one who had done similar exercise and can share the upgrades steps along with list of issues, gotchas , lesson learned etc
