Do I need a do Docker-Image of my BC on Prem for developing or is it possible to implement extensions without docker? My customer isn't a big fan of Docker on his systems.

You also develop direct on/into a Saas Sandbox, if it a small project

Docker just an option. You can locally install BC on-prem version and do DEV job based on this version.
