Just installed Insights 4.0.1 and get "Webpage not found" error

Hi I just upgraded to the newest insight and now the webresource on the form always shows an error with "webpage not found".

I updated the webresource for insights 4.0 following this guide to set it up on the form: https://kb.insideview.com/hc/en-us/articles/222846827-Enabling-Insights-4-0-for-Custom-Forms-in-Microsoft-Dynamics-CRM-Online

Anyone else run into this or know a solution/things to check?

    There is an error in the Parameter for the control - 

    solutionVersion=4.01&crm_version=v123

    Thanks for the reply! I just figured it out last week and realized that I accidentally did not name the webresource block correctly....

    So if anyone else has an issue out there and sees this -- be very careful!
