    Where is it getting that Id from ("DACE0EB1-D52E-469B-BA54-404300F8F2A9")? Is this what you are using in your code? Maybe it's just not a valid class id for the control?

    Classid is a new Guid id of the field attribute.How can we find class of the control?

    <row>

            <cell id=""{6e5975d3-64b5-14eb-00e4-064e3dd298b9}""

              <label description=""AttributeName""

                     languagecode=""1033"" />

             </labels>

             <control id=""new_bankname""

                      classid=""{DACE0EB1-D52E-469B-BA54-404300F8F2A9}""

                      datafieldname=""new_bankname""

    If it's classid, it should be the same for all controls of the same type.. if I were to do this, I'd probably just create a new form manually, add the controls I may need, export the form(as part of the solution), then copy paste classid-s from customization XML to your code.. then delete the form created above.
