I have the same question (0)Hello Pavan,
The Screen shot you have provided is showing a CRM 4.0 enviornment. As you have asked about CRM 2011, I have provided some brief items below, please let me know if any of these links below help you.
If you would like more information about the funcationaility and customizing CRM 2011 you can reference both our Implimentation guide and SDK resources available on the TechNet and MSDN. The SDK resouce will lists how to intergrate/customize different components of CRM 2011.

msdn.microsoft.com/.../gg309408.aspx
Also the CRM Blog Community page contains articles and steps that covers many parts of CRM 2011.

For creating a basic Webform in visual studio you can reference the following msdn article:
msdn.microsoft.com/.../aa728785(v=VS.71).aspx
Unfortuantely there is no built in Internet Lead Capture/WebForm for CRM 2011 on Premise. If you would like to avoid having to write your own custom code to intergrate with CRM 2011 our PinPoint team has all ready created a Lead Capture Page that can be easily intergrated into CRM. If you would like more details on this I recommend checking out the below link:
