I recently updated social Insights to the latest version. It obviously changed a lot, including the way that the information shows up on each record.
I had difficulty during the upgrade, but it's working now and showing up on Accounts and Contacts. However, it's not showing up on Leads.
I tried to add the module back in on the Form setup, but if I go to 'Insert', the Social Insights option is grayed out. I'm about to put in a support ticket. Anyone else have this problem?

Hm, that sounds pretty involved. I'm not a super techy admin, so I'm a little weary of this. The one thing I do have is a fair amount of form and view customizations, so I don't really want to lose those.
I'll put in a support ticket and see if I get lucky.
I haven't had this specific issue, but I know a good workaround for form issues like this is to turn on an unmodified instance of CRM, export a solution that is just the one entity that is giving you problems, and then importing it into your organization.
Fair warning, this also blows away other form customizations, and view customizations, but it might be worth it to restore your social insights.
Also, social insights might already be somewhere on the form, hidden behind a closed tab. I'd go tab hunting to double-check.
